WAIVER SHEET, RELEASE, WAIVER AGREEMENT
Cuyamungue Institutes Workshops

I understand and acknowledge that I am solely responsible for determining the extent to which I can
safely participate in any workshop, activity, or event, including physical, emotional, mental, and spiritual. In attending any activity, or visiting, on the grounds of The Cuyamungue Institute, in New Mexico, I agree to assume total responsibility for my own safety and well-being, and for the protection of my personal property.

I release from all liability and hold harmless The Cuyamungue Institute and its agents, including, but not limited to, Board of Directors, instructors, volunteers and staff, from any and all legal responsibility for any injury, illness, accident, loss, or other misfortune that may occur in connection with my participation in, or attendance at, workshops, activities, events, or visits, at The Cuyamungue Institute.

I further understand that under the terms of this Waiver and Release Agreement, I am releasing and
relinquishing legal rights that I might otherwise hold. I intend this waiver and release to apply to myself and to anyone who may have the right to make a claim on my behalf. This waiver and release is part of the consideration I give in order to enter the property of The Cuyamungue Institute for any reason.

As a workshop participant I understand and agree that the steps and process of the Cuyamungue Method may seem deceptively simple but that teaching the Method requires significant training and certification by the Cuyamungue Institute as an instructor. Completion of this present workshop is for my individual benefit only and I agree not to advertise and conduct workshops, seminars, presentations or otherwise teach the Cuyamungue Method to the public without becoming a certified instructor. Teaching the contents of this workshop to the public without certification – whether or not for personal financial gain – is in violation of this agreement.

However, I understand that I may share this method with others in informal gatherings where no fees are charged, using an audio recording of the rattle. The effect of live rattling on the nervous systems of participants requires that teachers be fully trained and approved by a mentor before leading sessions with live rattling.

We encourage any workshop students who are interested in teaching the Cuyamungue Method to inquire further into the in-depth certification training that is available and to become part of the network of teachers who are inspired to pass on these important teachings.
